weight management service for Rotherham Primary Care Trust.

Rotherham Primary Care Trust are looking to tender for the provision of
the following: - Lot I - Specialist Multidisciplinary (MDT) Childhood
Obesity Clinic (Tier 3); Lot II - Community Weight Management Service for
Adults (Tier 1); Lot III - Specialised Multidisciplinary (MDT) Obesity
Clinic for Adults (Tier 2). The Services will be provided within the
Rotherham area and tasks across the three services shall include
assessment, advice and information, basic intervention and prevention,
specialist advice and support, pharmacological therapy/intervention,
psychological support and onward referral where appropriate to other
support services. The main focus of the service provision is to provide
clients with appropriate intervention and support according to which
service they have been referred. THIS IS A PART B SERVICE AND THIS NOTICE
IS VOLUNTARY. Community health services. Health and social work services.

INFORMATION ABOUT LOTS
LOT NO 1
TITLE: UK-Rotherham: provision of a specialist multidisciplinary (MDT)
childhood obesity clinic in Rotherham area
1) SHORT DESCRIPTION: Provision of a Tier 3 Multidisciplinary Childhood
Obesity Clinic, providing specialist advice and support for children and
families where the child has a BMI indicating obesity in line with BMI
centile charts (above 98th centile). Community health services. Health and
social work services. Miscellaneous health services. Health
services.Health training services.
2) COMMON PROCUREMENT VOCABULARY (CPV): 85323000, 85100000, 80426100,
85140000, 85000000.
3) QUANTITY OR SCOPE: Referral of motivated client, meeting BMI criteria,
via GP (and after Tier 2 exhausted). Referrals will be from all supporting
practices across Rotherham. MDT approach for time limited programme of 12
weeks, including behaviour modification and advice, and support from
Registered Dietitian, and Physiotherapy physical activity advice for
clients, as well as psychological intervention. MDT led by specialist
practitioner (eg. GPwSI or GP with appropriate accredited training). Aim
client weight loss or weight maintenance in 12 weeks, maintained at 6
months.
Estimated cost excluding VAT: 770 000 GBP.

LOT NO 2
TITLE: UK-Rotherham: provision of a community weight management service
for adults in the Rotherham area
1) SHORT DESCRIPTION: Provision of a Tier 1 Community Weight Management
Service for Adults, for clients with a BMI 25-40. This will be a service
focusing on basic intervention and prevention of obesity, by promoting
healthy eating and physical activity. Community health services. Health
and social work services. Miscellaneous health services. Health services.
Health training services.
2) COMMON PROCUREMENT VOCABULARY (CPV): 85323000, 85000000, 85100000,
80426100, 85140000.
3) QUANTITY OR SCOPE: Clients will be supported and encouraged to lose a
minimum of 3 % body weight, aiming for 5 % body weight loss over a 10-week
weight management course, and clients will be empowered to manage their
own weight so that post course weight loss is maintained.
Estimated cost excluding VAT: 306 000 GBP.

TITLE: UK-Rotherham: provision of a specialist multidisciplinary (MDT)
obesity clinic for adults in the Rotherham area
1) SHORT DESCRIPTION: Provision of a Tier 2 Specialist Multidisciplinary
Adult Obesity Clinic, providing specialist advice and support for clients
with BMI >35 with co morbidities, or BMI >40 aged 15-75 years. Community
health services. Health and social work services. Miscellaneous health
services. Health services. Health training services.
2) COMMON PROCUREMENT VOCABULARY (CPV): 85323000, 85100000, 85140000,
80426100, 85000000.
3) QUANTITY OR SCOPE: Referral of motivated client, meeting BMI criteria,

evidence of Tier 1 intervention prior to referral. Referrals will be
received from all supporting practices across Rotherham. MDT approach for

accordance with the legal provisions of the country in which he is

(g) is guilty of serious misrepresentation in supplying the information
required under this Section or has not supplied such information;
(h) has been the subject of a conviction for participation in a criminal
organisation, as defined in Article 2(1) of Council Joint Action
98/733/JHA;
(i) has been the subject of a conviction for corruption, as defined in
Article 3 of the Council Act of 26.5.1972 and Article 3(1) of Council
Joint Action 98/742/JHA3 respectively;
(j) has been the subject of a conviction for fraud within the meaning of
Article 1 of the Convention relating to the protection of the financial
interests of the European Communities;
(k) has been the subject of a conviction for money laundering, as defined
in Article 1 of Council Directive 91/308/EEC of 10.6.1991 on prevention of
the use of the financial system for the purpose of money laundering.

capacities of the supplier or the technical capacity of the service

(j) an indication of the proportion of the contract which the services
provider intends possibly to subcontract;
(k) with regard to the products to be supplied: (i) samples, descriptions
and/or photographs, the authenticity of which must be certified if the
contracting authority so requests; (ii) certificates drawn up by official
quality control institutes or agencies of recognised competence attesting
the conformity of products clearly identified by references to
specifications or standards.
